Actually, I've found that Drawde/Telmets Combined Working Database has Plane of Nightmare, Plane of Justice and one or two others with spawns in it. It's a combined database so there's lots of bugs...

Make sure you replace the username and userpass with whatever you want your login and password to be. Then save in your EQEmu directory under miniloginaccounts.ini
