double check the conf files in the PEQ directory.
i was having all kinds of opcode problems before and the thing that fixed it was copying the patch_6.2.conf file and replacing opcodes.conf with it
try that and see if it helps, just rename the original opcodes.conf to like _old_opcodes.conf before doing this so you can roll back if needed
