According to your picture....

Go to D:\Program Files\Sony\Everquest

Find the Eqhost.txt file. Delete it. Start eqgame.exe again (Make sure its a shortcut with the -patchme extension on it) and it will make a new eqhost.txt file and should load properly. On a side note, Running EQ in windowed mode will perminantly aliviate this issue. There is a bug where running EQ in fullscreen will bug out your game and cause this error to occur if it shut down improperly, or crashes or what ever in fullscreen. Windowed mode is the way to be.
