As Jahar said - We are using all of the Diablos, one, two and three. Three we are only using the classes, because lets be honest, Diablo 3 SUCKED. xD
Hmm, I'm a bit confused on what you mean far as the gear, but lets throw this example out there; SwordA has 10 damage, 25 delay, and random stats (Example; 1 STR, 2 STAM and 10 HP). SwordB has 8 Damage, 25 delay, and random stats (Example; 2 STR, 3 STAM, and 15 HP). -- Mind you, as I stated, there are over 1,000 CUSTOM items made, while I will not tell you how many of each type can drop, I can assure you, it's plentiful (We'll add more if we have to.

Both of these items are Item Level 10 -- As said every *normal* item is Item Level 10.
If you kill a boss, lets use Leoric;

You walk in, kill Leoric. Once you do so he has a chance (80%) to drop a RARE item, a chance to drop a LEGENDARY item (10%), and a chance to drop a SET item (.1%), as well as a couple of normal items. The RARE is Item Level 11, Legendary is Item Level 12, and Set is 13. This way we can have a randomized chance of the boss dropping each of these items. -- When you advance to the next part of the act AFTER you kill Act One Part One boss the item levels go to 15, so the next big boss will drop the same set up, except, item levels will be 15. So in theory, any boss mobs that we have set up, has a chance to drop Rare, Legendary, or a Set item.

We used GeorgesS tools top randomize all of our stats (Which by the way, was awesome.)
