I've had this problem on my server. I usually set them to 1 level higher than they were when they lost their levels. So if they died at level 93, then I set them to level 94. Also, if they are/were max level, then I delevel them by 1 level, then level them up by 1, and that fixes the problem too. I dont know why it happens, but I've seen it now about once a month. I've also noticed ghost logged in at same time, like the same account logged in twice, so dunno if that was a problem. I just did a #kick to get rid of the player + his ghost, then fixed the level problem, and never hear about it again from that user. Dunno the cause, but thats how I fix it.
