EqEmu is now compatable with a Fresh install of the Titanium Package. Before you run the game though you should change your Desktop Icon (Shortcut to EQ) to point to eqgame.exe with the patchme option at the end.

So path to Executable would look like this:

c:\EverQuest\eqgame.exe patchme

This will bypass with Patcher and go straight into the game. I hope this is clear enough.
