Default DW and Triple/Quad attack flag

Question to those who knows how it all works.

the facts:
-the mobs do not get DW on Emu until lev 45 (no one knows why), unless they monks.

Which means they can only Double Attack.

Now there are Triple and Quad attack special flags.

If you get a mob under 45 a triple attack- it will triple- its simple.
However what happens to mob above 45?
A lev 45 mob has DA and DW- which means it will have 4 attacks. with a triple attack flag - how many extra attacks he receives? 3 from each hand? or just +1?
What about Quad flag?

Also, does mobs follow same rules as players for using their skill cap when performing DA/DW attacks? For example the fact that Ranger has both DA and DW by 20, that doesn't mean he can instantly hit 4 times- the skill cap at 20, is so low, that he only gets about 50% chance to DA, 50% chance to DW, and only like 10% chance to DA with offhand. Only by level 50 or so he starts to produce more or less stable 4x attacks output each round.

Still curious why normal mobs do not get DW till 45...
