I did a bit more testing and researching last night and think I have some information that may help you guys figure out what is happening on my end. Hopefully the length of this thread hasn’t turned people away from viewing it. I feel we are really close now to figuring out what simple step or file is missing.
Too keep it simple I one tackle one problem at a time. Maybe solving this will solve the other poblems.
On the problem of the LDON Shard of Eternal Pain, none of my fresh installs had the proc working correctly but it did work in AKKA’s install. I took and did a search in his database for SwarmPetLDONSar, and then mine, and noticed some key differences. He had entries in Pets and NPC_types and mine did not. I only had the actual item and the proc entry in spells_new.
After this I loaded up all the original source files, that I know of, and did a search in notepad++ to see if any of these files had it entering the data in Pets and NPC_types to show where I went wrong and I have yet to locate that file. I did see where the original peqbeta_2015-08-12-02 file was inserting the data into spells_new but no entries for Pets or spells_new.
Did any of that trigger an ah hah moment on where I might consistently be failing?

If you are using the new Akka auto install, it pulls the latest DB daily/weekly dump from PEQ. I do not use that database when I install, I drop it and download the current DB thats available here: https://drive.google.com/folderview?...p=sharing#list
As I found that the current PEQ DB daily dumps have some errors and omissions, as well as it may have enabled/disabled whatever Cavedude is running on the Grand Creation at the current time. As an example I did this during halloween and the Halloween event over on PEQ was running, but there was no instruction on what queries to run to disable it. So in my digging, I realized I was not getting the above DB but the daily dumps.

peqbeta_2015-08-12-02_01.tar.gz is the file I did use. It was last created 8-12-2015. Perhaps, the daily dump you mention has it but th 8-12-2-15 version does not. If you use the peqbeta_2015-08-12-02_01.tar.gz version, does SwarmPetLDONSar show as an entry in Pet in your database? If not then that problem explains a lot.
EDIT: I install my own server from instruction on the Wiki. I use AKKA in a different VM to compare things like the problems I have having to see if it is a global issue or just an issue with me. Ultimately I am not sure what is supposed to work, what has been broken by code changes and what hasn't been fully implemented yet. I test first on my end, do a bit of research then I post. The problem I am noticing, just like your example, is everyone is pulling from so many locations and different version of files are being used. So, it keeps things... interesting!

If I am not mistaken, I do not think LDON and anything associated with it actually fully works. I have never seen it work without a lot of "fixing". Most of those folks that have it fixed never really chime in on it. I think the mechanics are there, but if you are under the premise any of of is supposed to "work", you are moving in the wrong direction. You more than likely will have to fix it yourself and hopefully share, or get one of the uber coders on here to fix it with you. But I am really not seeing those guys that have it working helping. In fact, Demonstrar says in an above post some of it is not working.
As an example, PoP progression is suppossed to be fully functional, but its not. There are scripts that do not work and have to be recoded. I recoded many up to tier3 and got tired of messing with it. Same for LDONs as well.

I did run into some progression issues as well. I had to work around them. Sadly, I have practically zero programming skills. A friend and I will be messing with GoD progression stuff. Is that somewhat functional?

As far as I know none of GoD works. I am pretty much stalled on a server as well, as I need a lot of time to learn what to fix and then fix it. Example was learning enough perl to troubleshoot the scripts. Sadly, right now I do not have time to spend. There are some awesome people on here that will help if asked. What I would like to see if everything truly functional up to at least PoP. Everyone says it is, but in reality its not. Reality is running a server is a lot of work!

I believe what I need to do is first find out where to grab the latest version of stuff the main devs are working on. Then slowly figure out what is or isn't implemented, then I can start learning and playing around with stuff. I just need to make sure what I have created is current and running as designed.
People like you and I need to form up a tracker page. Something that allows us to keep track of what isn't implemented or is broke. Maybe we can then start working together to improve the content. If something like that does exist, let me know so I don't annoying everyone here every couple days asking if X is supposed to be working or not.
EDIT: I do want to somewhat correct myself concerning PoP progression. There were a couple of keys I did skip, as the NPCs weren't responding, but as a whole most of it worked well. The tribunals were broken for me except for Flame and maybe one other. Summoning the stones and giving them to the Executioner gave a flag, as a work around. I did notice that having bots in your group often bugged the quest givers. They would not respond or would only respond to a certain point. If you got rid of the bots and repop the zone, they went back to working. Repopping the zone alone did not work. This may be something a lot would run into as I believe bots are somewhat popular. I believe I had an issue with the Behemoth not spawning anything after death. That i would have to check again to be exact.

The expedition system itself has not been implemented.
I believe there are a couple of pseudo-expeditions coded into scripts..but, that won't be the final product.

Alright here is my conclusion on this thread, for those who have similar problems. It seems there was no corruption with my database and no issues with my installation.
1. Charms - It appears most charms are broken and do not function. I found 2 that work after you zone. The rest nothing. I had to work around this by modifying the charmfieldid and setting it to 0. This maxes the charm sadly, but at least it works.
2. Unknown DB - I did copy over the generated spell_us.txt and someone gave me a dbstr_us.txt file. This corrected 90 percent of my errors. I would love to find a way to complete it but this is at least very usable now. No installation appears to generate a dbstr file. One person told me that imported those into their server, not generated. My problem is having a solid one to begin with. I guess it is possible some of the unknowns are that, unknown.
3. LDON raids. It appears a majority of these have not be implemented yet. I do not have a current list of the ones that are. I may update later if I get one.
4. Shard of Eternal Pain - This is broken using the current file that the wiki links you to. I had to download the weekly update and it works there.
Hope this information proves useful to someone.
EDIT: Just to clarify on the charms, I did not test them all. I only tested roughly 12 different charms of 65 level or lower. The charms I am talking about are the ones that scale based on different conditions. Not charms that have static stats outright. I just realized this might have been part of the confusion for some.
