You've enabled a buttload of inbound ports, but never turned on any outbound ports.

EQ needs these to function:

Login - you being able to establish an outbound UDP to 5994-5995
Connect to a world server - establish an outbound UDP to 9000

The individual zone ports can vary by server. Check with your server op to see what you need to turn on there.
