Sorry to bump an old one, but I wanted to show I've at least done my research while having a similar problem. In fact, I've used the search function and checked the four threads that contained this OP Code error and six pages of threads regarding zone.exe crashing.

Anyway, down to business:

I'm getting an obscene amount of zone crashing lately, and this error is found either immediately before the zone starts to go down, or a few lines further up. It is consistent, and I really feel it's the smoking gun to a problem that's been plaguing me for months. However, I'm unsure of exactly what it means, what is causing it, and how to fix it. Which is where you (hopefully!) come in.

It is quite common to see in the logs, and doesn't always seem to result in a crash. However, it's the only error of any kind that surfaces consistently when zones go down. The only other trouble spot I can note appears to be hundreds upon hundreds of lines of this with no break in between:

This error has occurred with both dynamic and static zones. I haven't confined it to one single character or class or anything really decisive. The four characters above are (in sequence) warrior, bard, cleric, and shadow knight, so it seems it can occur with anyone.

Server is running Windows Server 2003, 4 GBs of RAM, and runs like a dream other than this one little hiccup. We're running the current PEQ database and current build (1090).

This is just making for a depressed player base, and making me feel like quite a let-down to have no answers for them. I'd be thrilled with any suggestions to try. Thanks for your time.
