I'm running Rev 386 atm (maybe a little higher) and updated about a month ago. The rules errors started appearing after those updates. I double checked the changelog and inserted everything I could find.

The only custom things on my server are quests/items/npcs. I have modified rules and database entries but nothing outside of standard allowable parameters.
