If LoH and HT are refreshing each time you zone, and are truly refreshing (as in you cast cast them fully as opposed to having them interrupted with "Spell recast not yet met" or whatever it says) it's more than likely a spellfile issue, but could also be an AA table issue.

I would check recast/recovery times in the spellfile first before I dig around and mess with any AA's because AA's aren't as simple.
