I was having the "you died" bug on Freedom Alliance the other day. I logged my toon in fine, changed zones and died immediately on zoning. From then on, I die soon as I zone in. We're pretty sure it has to do with Lore items.. i'v had this happen a few times to me as well and the only way to fix it right now is remake character :(
