Just wanted to add.

4. Your account doesn't exist on the SQL server *either EQEMU's or your own*

or your sql database doesn't exsist at all. make sure you install an sql before trying to run your server.
or you don't have access rights on your server, make sure you grant your server access rights into your database, and that the correct user/pass is in the db.ini file
